Medical services are delivered by licensed independent physicians contracted with QIM Health, operating under their own state medical licenses.

Information you provide
When you create an account, complete a health assessment, schedule an appointment, or contact us, we collect your name, email, phone number, date of birth, address, and payment details. If you become a patient, we additionally collect health information you provide in intake forms, questionnaires, and consultations, as well as records generated during your care.

With your authorization, we receive information from laboratories (biomarker and test results), pharmacies, insurance companies, other healthcare providers, and third-party identity-verification services.

7. Cookies & tracking.
We use cookies and similar technologies for essential site functionality, analytics (Google Analytics 4), conversion tracking (Meta Pixel, GoHighLevel pixel), and — where applicable — marketing personalization. Disabling non-essential cookies will not affect your ability to use the telehealth service.
